Leaks start at rod seals and fittings. Keep rods free of grit, torque fittings to spec, and replace weeping seals before they take the tube with them.
Use the right fitting standard (SAE / BSPP / ORFS) for the equipment; mixed standards are a common leak source.
A documented inspection and test report at build time catches the majority of issues before they reach the field.
